To begin the process to order, complete this order form. Once building level approval is granted, scan the order to the Director of Facilities for approval. Once approved, this requisition will be returned to the bookkeeper to complete the purchase order. Once an approved purchase order, is generated, either email your order form and approved PO to JK Smith. Email is JK.Smith@coastalsanitary.com. Coastal Sanitary will not process any orders without an approved PO. Orders will be shipped directly to your location.

Emergency Procurement Form
The proposed procurement action is being procured in accordance with Board Policy DJ –R 2-106 of the District’s Procurement Code. An emergency procurement may be made when an emergency condition arises and the need cannot be met through normal procurement methods provided that, verbal approval by the superintendent, procurement director or designee of either shall be obtained prior to the procurement.
