Medication Procedures

Medication must be brought to school by a parent or guardian in the original container.

A Medication Permission Form must be completed for each medication provided to the school.

Forms are available in the school nurse’s office.

Parents must sign the form even if the student is over 18 years old.

A new Medication Permission Form is required each school year.

Prescription medications require a physician’s signature on the Medication Permission Form.

The school nurse may assist with obtaining this signature, but it is the parent’s responsibility.

Non-prescription (over-the-counter) medications require only a parent’s signature.

Students may not bring medication to school without a signed form on file.
-
Controlled prescription medications must be counted by a school employee and the parent at the time of drop-off.

School staff cannot dispense medication without a signed form—even if contacted by phone.
