Medication Procedures

  • Medication must be brought to school by a parent or guardian in the original container.

  • A Medication Permission Form must be completed for each medication provided to the school.

  • Forms are available in the school nurse’s office.

  • Parents must sign the form even if the student is over 18 years old.

  • A new Medication Permission Form is required each school year.

  • Prescription medications require a physician’s signature on the Medication Permission Form.

    • The school nurse may assist with obtaining this signature, but it is the parent’s responsibility.

  • Non-prescription (over-the-counter) medications require only a parent’s signature.

  • Students may not bring medication to school without a signed form on file.

  • Controlled prescription medications must be counted by a school employee and the parent at the time of drop-off.

  • School staff cannot dispense medication without a signed form—even if contacted by phone.
